2014 Scholarship Recipients

The Health Alliance of Clinton County (HACC) once again awarded two $2,000 scholarships to two Clinton Memorial Hospital employees to further their education in healthcare.

The recipients for 2014 are May Phipps and Evan Penn. May is a senior at Galen’s School of Nursing and will graduate in June 2015.  Evan graduated from Galen in October 2014 and is continuing his education at Ohio University for his BSN.

 

PURPOSE:    Two medical scholarships will be awarded yearly by The Health Alliance of Clinton County. These scholarships will be awarded to:

Employees of Clinton Memorial Hospital,
Employees of Doctors’ offices within Clinton County
Employees of Clinton County Community Care Hospice

ELIGIBILITY:   The HACC Scholarships are open to all Clinton Memorial Hospital Regional Health System employees, to employees of the Doctors’ offices within Clinton County and the Clinton County Community Care Hospice who have worked at that place of employment for at least one year.

SCHOLARSHIPS:   Two Medical Scholarships in the amount of $2,000.00 each will be given in 2015.  Scholarships will be awarded to an employee pursuing a nursing degree, an employee who wishes to become certified in their field, change fields of interest or to acquire a BS/MA in their chosen field.  This employee must continue to work with the said employer during the period of the scholarship or the eligibility will cease, unless employment is terminated under no fault of the recipient due to dismissal or layoff.  Should the recipient terminate his/her employment during the period of the scholarship, the recipient agrees to repay the scholarship amount received to The Health Alliance of Clinton County upon request of the committee.

The committee reserves the right not to award a scholarship if there are no qualified applicants.

The recipients will be required to submit documentation from the college/university of expenses to be incurred: his/her tuition, registration, general fees, and lab fees of which up to $1,000.00 (1/2 of this yearly scholarship) will be paid directly to the financial office of the college/university per semester. For any remaining scholarship funds over the amount paid directly to the school, the recipient may submit receipts for other expenses such as books. Travel expenses are not eligible.

Requests for payment must be submitted to The HACC Scholarship Fund, c/o Mrs. Patricia A. Richardson, 283 Briargate Drive, Lebanon, Ohio 45036.

Any previous recipient of either scholarship may reapply for another scholarship but will be required to compete against a new field of candidates.

Please submit all completed applications to HACC before the deadline date of 6/15/15.

The HACC raises money with their Book Fairs, $5 Jewelry Sales and the annual Holiday Ball and Auction.  New additional fund raisers are always being considered.     A portion of the fundraiser’s proceeds is given to the Scholarship Fund to help replenish the fund for future scholarships.
